چکیده

A34-year-old woman presented with recurrent 15-minute episodes of palpitations, lightheadedness, and chest tightness. ECGs obtained during these episodes revealed striking global T-wave inversions that resolved spontaneously 2 hours after each episode (Figure 1). Echocardiography revealed severe left ventricular dysfunction with an estimated ejection fraction of 20%. Cardiac catheterization demonstrated angiographically normal coronary arteries. A 24-hour urine collection for catecholamines showed an epinephrine level of 227 mg (normal, 2 to 24 mg) and a metanephrine level of 3803 mg (normal, 95 to 475 mg). Abdominal/pelvic MRI identified a right adrenal mass (Figure 2). I-MIBG scintigram showed marked uptake in a single location corresponding to the right adrenal gland (Figure 3). The patient was diagnosed with an epinephrinesecreting pheochromocytoma and underwent an uncomplicated right adrenalectomy (Figure 4). Three months after surgery, the patient was asymptomatic. A repeat echocardiogram revealed normal ventricular function, and urine catecholamines remained negative.

Pheochromocytoma Presenting with Multiple Cardiovascular Manifestations

Excess concentrations of catecholamines in pheochromocytoma can cause various clinical manifestations. There are few reports of pheochromocytoma presented with stress-induced cardiomyopathy (catecholamine-induced cardiomyopathy) after mild stress and the case of pheochromocytoma associated with nonocclusive stroke is more uncommon. We report a case of pheochromocytoma manifested repeated stress...

A vicious cycle of acute catecholamine cardiomyopathy and circulatory collapse secondary to pheochromocytoma

Acute catecholamine cardiomyopathy is an uncommon, life-threatening manifestation of pheochromocytoma. The massive release of catecholamines from the adrenal medulla and their toxic effects on the coronary vessels and the cardiac myocytes play a significant role in the pathogenesis of cardiomyopathy in patients with pheochromocytoma. Severe manifestations, such as acute catecholamine cardiomyop...

Pheochromocytoma-induced cardiomyopathy is modulated by the synergistic effects of cell-secreted factors.

BACKGROUND Pheochromocytomas are rare tumors derived from the chromaffin cells of the adrenal medulla. Although these tumors have long been postulated to induce hypertension and cardiomyopathy through the hypersecretion of catecholamines, catecholamines alone may not fully explain the profound myocardial remodeling induced by these tumors. We sought to determine whether changes in myocardial fu...

Recurrent Catecholamine-Induced Cardiomyopathy in a Patient With a Pheochromocytoma

Pheochromocytomas presents with variable clinical manifestations. Cardiomyopathy caused by a pheochromocytoma is well known. We report the case of a 62-year-old woman with recurrent left ventricular dysfunction, who was subsequently found to have a pheochromocytoma. The patient had two different patterns of cardiomyopathy. Patients with a cardiomyopathy, of non-specific origin, should have a ph...

Captopril improves hypertension and cardiomyopathy in rats with pheochromocytoma.

Hypertension and cardiomyopathy are prominent findings in humans and rats harboring pheochromocytomas, tumors that can secrete enormous quantities of catecholamines. We have previously found that alpha- and beta-adrenergic receptor antagonists may ameliorate the hypertension and cardiomyopathy found in New England Deaconess Hospital rats implanted with pheochromocytoma. The present studies were...

Case Report : Pheochromocytoma - related catecholamine cardiomyopathy with successful outcomes after orthotopic heart transplantation

Pheochromocytomas (rare catecholamine-producing neuroendocrine tumors) have many different manifestations, and complications can occasionally include myocardial infarction and cardiomyopathy. In a majority of cases, cardiomyopathy reverses following medical or surgical treatment of the pheochromocytoma. We report a case of a 28-year-old male patient with preoperative diagnosis of pheochromocyto...
